    Patient Care Coordinators/Medical Records
The Patient Care Coordinators check patients in for their appointments, check status for information updates including insurance / finding alternate resources for payment and make future appointments including processing referrals. The Medical Records Technician process both requests for patients needing their personal record to move with them to other facilities and records that are coming into our facility. 


    Behavioral Health
YTHC's Behavioral Health offers a holistic approach to behavioral problems, including substance abuse counseling, mental health counseling, stress management and prevention education.  See additional information under the Programs tab on this site.


    Community Health
YTHC's Community Health & outreach programs include Public Health Nursing, CHR Services, WIC/Nutrition, Health Education, Senior Meals, Transportation, Elder & resources for local caregivers.

    Prevention / Circles of Care

    Diabetes Education Program
The program includes group classes, support group & individual education appointments.  Patients are encouraged to bring a family member or friend when attending.  To schedule an appointment with our  Diabetes Educators call 541.278.7543.  To see the 2012 schedule of diabetes education classes & support groups please see the Calendar of Events tab on this site.  The Diabetes Education Program is part of a team of educators at YTHC that help patients build skills to prevent complications & stay healthy.  To learn more call the department directly at 541.278.7543.   


    Stepping Out Program
Yellowhawk Stepping Out is a walking program designed to improve mobility, provide cardio, and increase stamina. YTHC Stepping Out is an honor based exercise program for individuals to walk or exercise equivalent to 100 miles or more in 6 months.  Participants log their miles and then turn in their completed log to Beth at the end of the 6 months for a small reward....


    Exercise Program
YTHC's Exercise Program (YEP) was developed to support YTHC patients 12 years of age & older that have a desire to improve their health by focusing on exercise & nutrition. Participants have the opportunity it work with a Register Dietician, Certified Diabetes Educator, Behavior Consultant & Personal Trainer.


    Dental
The Dental office provides regular diagnostic and prevention services as well as routine clinical exams, cleanings, restorative & specialty work for eligible patients.


    Medical
The Medical Department team is comprised of health care professionals with a large array of education and experience. The primary focus of care is family practice which covers patients of all ages.  Our approach is to drive, provide, and educate preventative care services. Yellowhawk takes pride in providing quality health care to our patients.


    Pharmacy
The Yellowhawk pharmacy provides general pharmacy services including prescription medication services, adult immunizations, blood pressure screenings and diabetic glucose meter instruction.


    Executive Management Team
Yelllowhawk Executive Management Team is responsible for the overall, day-to-day operations of the programs and services, operationalizing  strategic and annual work plan objectives, and continually striving for quality improvement.


    Business Office
The Business Office at Yellowhawk oversees the Contract Health Services funds as well as the capturing of current patient data. This data is reviewed to determine eligibility for services and third party billing information.


    Finance
Responsible for the overall financial administration of Yellowhawk programs and departments.


    Human Resources
Yellowhawk Human Resources (HR) focuses on recruitment, providing direction, compensation, performance management, organization development, safety, employee motivation, communication, administration, and training for the people who work within our organization.
