The CTUIR Health Assessment is a survey that takes a snapshot of the health status of our community.  We use this information to create our priorities for Yellowhawk’s strategic plan and annual work plans. Please participate and encourage patients and community members to participate. The survey is for 18 years and older CTUIR tribal members and community members. This survey is anonymous, so please be as truthful as possible. There will be a chance at the end to enter your contact information. The survey will take about an hour to complete. Click HERE to take the assessment.
