Chemical Dependency Counselor

Provides outreach, prevention, and outpatient chemical dependency counseling/education to patients served by the Yellowhawk Tribal Health Center. Includes diagnostic assessment and documentation for each client on services and activities provided.

Hiring Salary Range $41,435-$51,852

Maintenance Tech/Housekeeper- Position Description

Medical Director

Responsible for leadership of direct medical care operations. Requires experience and skills to assess and employ clinical competence, current best practices and process improvement, along with responsibility for the overall performance of physician and non-physician providers, including locums and contract providers.  The Director will spend about 70% of the time in administration of the medical program(s) and about 30% of the time acting as a physician and consultant to medical providers.

Medical Director- Position Description

Chemical Dependency Clinical Manager

Provides support in the planning, development, execution and on-going evaluation of Yellowhawk Tribal Health Center Behavioral Health Chemical Dependency programs and clinical services, assuring that they all meet appropriate professional standards, licensing/certification and grant conditions requirements. Responsible for developing and maintaining outpatient services which include, but are not limited to outpatient, intensive outpatient, DUI client assessment, and alcohol and drug information school.  Is able to teach alcohol and drug information.  Is also responsible for assuring that the Chemical Dependency health programs and services are culturally proficient for the populations served.

Hiring Salary Range $54,780-$68,485

CADC Clinical Manager- Position Description

Mental Health Clinical Manager

Provides support in the planning, development, execution and on-going evaluation of Yellowhawk Tribal Health Center Mental Health programs and clinical services, assuring that they all meet appropriate professional standards, licensing/certification and grant condition requirements.  Must have extensive knowledge of mental health disorders, therapeutic interventions, knowledge of working with families, adolescents and children and demonstrate cultural competence and be highly skilled in best practice and evidence based counseling techniques.

Hiring Salary Range $72,453-$90,592

Mental Health Clinical Manager- Position Description 

Human Resources Director

Under limited supervision, oversees the administration of Human Resource (HR) programs and operations for all employees.  Responsibilities include oversight of employee relations, intake and records, provider credentialing, recruitment and onboarding of new employees, and oversight of projects and initiatives that contribute to a productive, satisfied workforce. Includes oversight of Workforce Development grant staff and requirements.

Hiring Salary Range $95,896-$119,892

Human Resources Director-Position Description

Integrated Health Care Coordinator

Position works collaboratively across systems to provide overall management and coordination of a patient’s recovery care plan for individuals with substance abuse disorders, medical needs/chronic health issues, and co-occurring disorders in an integrated care setting. Position is grant-funded through October, 2020.

Hiring Salary Range $41,435-$51,852

Registered Nurse (RN)-Position Description

Senior Integrated Recovery Coordinator

Provides dual diagnosis and substance use disorder services (SUDS) within a primary care setting  by completing assessments, screenings, and providing cognitive and behavioral interventions. Collaborates with the primary care teams to enhance understanding of patients’ needs to plan and implement treatment, then monitor progress. Oversees daily activities of the BH21 grant and assists the Behavioral Health Director with data collection, grant administration and implementation.
