Patient Care Coordinators greet and check in people for their appointments, manage scheduling, process patient referrals and ensure that all necessary contact information is up to date.

Our Patient Care Coordinators also collect and update data that is required by the federal government in order for our community to receive its treaty right to health care.

Some of the information that Patient Care Coordinators are required to collect at least once a year is as follows:

  • Contact phone number(s)
  • Proof of residency (click here for more information)
  • Emergency contact
  • Insurance information
  • Family members living in household
