Community Health Plan

Community involvement in our planning is critical. The Community Health Plan combines our independent Community Health Assessment (CHA) with a 5-year Community Health Improvement Plan (CHIP).

Yellowhawk and our partners completed the 2024–2028 Community Health Improvement Plan (CHIP). A CHIP cannot be accomplished without the feedback and perspectives of our community.

In December 2022, Yellowhawk completed our very first, independent Community Health Assessment (CHA). Our Public Health Department leads the CHA and CHIP process. In late spring, 2026 Yellowhawk Public Health team started on the second CHA! Results from that survey will follow for the next CHIP.
