Community Health Plan

Thank you for your support in our Community Health Planning.

Community involvement in our planning is critical!

 

Yellowhawk and our partners are currently in the process of leading and creating a Community Health Improvement Plan (CHIP).

 

A CHIP cannot be accomplished without the feedback and perspectives of our community. Please guide us in creating this plan by answering our survey questions and/or attending an in-person community feedback session. To learn more about the CHIP, please visit our CHIP page.

 

In December 2022, Yellowhawk completed our very first, independent Community Health Assessment (CHA).  To review the data our community provided for the community health assessment, or to simply learn more about what it is and why it’s important, please visit our CHA page.
