Home visits are linked to a plan of care and are tailored to individual needs. CHRs may assist with:
- Vital signs
- Health education
- Connections to resources
- Assessment of additional needs

CHRs provide health-promotion and disease-prevention education in community settings such as:
People who are homebound or have difficulty leaving home may contact the pharmacy and request that medications be delivered to their home.
Medical transportation is scheduled in advance. A CHR may attend the medical appointment with the client if the client wishes. Family members or caregivers are strongly encouraged to attend when appropriate.
CHRs also help with planned, scheduled trips related to food security and access to important resources, such as:
CHRs provide in-home assessment and education related to fall-risk factors such as medications, chronic conditions, balance, physical abilities, and the home environment.
CHRs can assist Elders with a history of falls, near falls, or risk of falls with access to the Connect America medical alert system, which provides continuous in-home and mobile monitoring.
